are medically ready for discharge
cannot afford care
have no income or family support
have no safe home to return to
require temporary placement before long-term solutions
Who This Helps
- Hospital identifies a suitable patient
- Project D.E.O screens and verifies eligibility
- Sponsorship / subsidy is matched
- Patient is placed into a registered care centre
- Patient receives the necessary care until long-term arrangements are made
